Appointments can be scheduled Monday through Friday. Our phones are available for your convenience Mon-Thurs 8am -12pm and 12:45pm – 4:30pm, Friday 8am-12pm and 12:45pm – 4pm. Our phone number is 616.392.5973.
Appointments
Referrals
A referral from another physician is not required for a yearly gynecological exam. When you are referred, however, our physicians will send your referring physician an exam report.
Fees and Insurance
Please bring your insurance cards with you to every appointment.
We participate with most insurance companies. Please contact our billing office to determine whether we participate with your insurance company. Refer to our financial policy for more information.
Medical Records/FMLA/Disability Forms: We charge an administrative fee for processing medical records, FMLA forms and disability forms.
No Show or Cancellation
It is very important that you keep your appointment with us. If you don’t let us know 24 hours in advance that you need to cancel your appointment, we may be unable to contact another patient who needs to be seen and could benefit from an opening in our schedule.
Discontinued Services
Our physicians reserve the right to dismiss a patient from the practice if the patient is continuously non-compliant, combative or disrespectful to physicians or staff. We also have the right to terminate the physician-patient relationship due to failure to pay for services.
